I hereby release the above named organization, their affiliates, employees, volunteers, and those individuals participating in the above event, from liability and claims resulting from injury, illness, or damages which my child may suffer while at the event. I understand that participating in the Friends of Scott Prom is a privilege and I promise to behave appropriately.
